Questions and Answers on USP 797

Questions and Answers on USP 797: Answers Provided by Eric S. Kastango, MBA, RPh, FASHP:

  • E25 Question: Are there any guidelines regarding the commonly used 10% rule? (i.e. if adding an additive to a base fluid where the additive’s volume exceeds 10% of the volume of the base fluid, the additive’s volume is first withdrawn from the base fluid, before the additive is added).

  • E25 Answer:  I am not aware of this 10% rule that you reference.  It is a dangerous practice to use 10% rule, rule of eight and other mythic practices that permeate the practice of pharmacy.  Depending on how critical it is that the final volume be exact will determine how to handle the bag of fluid.  IV bags are not consistently overfilled and the overfill varies between lots of product.
